Thank you for choosing our center to care for your child. If you have been directed to this page, then our classrooms are currently full. Please fill out this form in full and submit it to our office to place your child on our waiting list. As soon as an opening occurs, you will be contacted directly. At that time you will be asked to fill out additional forms. These forms will be mailed, faxed or issued in person to you. As soon as you fill them out, please call to set up an appointment to visit the center if you have not previously done so. I will be happy to show you the facility and go over the programs, policies, and procedures. Upon admission, you will receive a weekly schedule detailing your child's specific curriculum goals.

While we do not know when an opening will occur, I want to assure you that every effort will be made to get your child into the program as soon as possible, according to his or her standing on the waiting list. If you would like, you may phone periodically and check on the status of the waiting time.
